The year after giving birth remains a risky time for mothers: most of the pregnancy-related deaths occur during this time period. Much of the efforts to improve pregnancy care for women have concentrated on the time between conception and giving birth, medical care after giving birth remains fragmented.

Advice from the multidisciplinary team (MDT) meetings therefore routinely includes specific post-natal management advice.
More broadly the EMMMN has started to work with GP representatives to look how communication between maternity units and surgeries can be improved. Specific initial work will focus on:
- Personalised management plans with bespoke timing and responsibilities for follow-up for women with medical conditions
- Hypertension pathways
- Gestational diabetes pathways
- Care of women who require blood thinning medication
